Season 8, Episode 11, 'My Soul on Fire, Part 1'. In the Bahamas, after Elliot walks away, an old guy with a drink walks by and says "She's all mad." I'm sure it is a cameo, but who is the guy?

No. Much older. Definately not an actor. Voice sounded familiar. Cameo didn't last more than two seconds. Dick Dale, perhaps? One of the Beach Boys?

I'd say it's a member of the non-acting crew of Scrubs. We saw Deb Fordham in "My Full Moon" doing the low-arm pump, and there was a joke involving some other writers earlier in the show. "She all mad" has definitely been said before in the show, I think by Laverne, so maybe it was an homage to her. Just a guess.

Maybe it was a scene similar to one that happened in the hospital in a previous episode - and this was a recreation of the same scene in the Bahamas, with the guy in place of Laverne???

It's quite similar. Laverne says it in My New Old Friend when JD and Elliot have just stopped being sex buddies but JD is still in love with her. Elliot has all her stuff stolen and really needs a friend, but JD isn't there for her because he's confused by his relationship with Elliot and in stead of helping her goes on a date to make her jealous. Laverne witnessed their argument about it and described the events to her husband over the phone, saying 'Now she all mad.'

According to Scrubs HQ, they couldn't afford to take any more guest stars to the Bahamas, so Bill Lawrence cast himself, and the other parts were filled by locals. So it's just some guy.
